Physio Consultation Process
If you haven’t had treatment with me before, please call or book an initial chat with me by phone so I can understand your physical health concern or injury.
Your initial consultation will include a full written case history, including your general health, any hereditary conditions, medication taken, past and current fitness levels, previous injuries/operations/illnesses and the reason for attending.
Reasons include treatment of an injury, injury prevention, maximisation of rest and recovery from training or to improve flexibility and reduce tension. It's important to establish why you're attending and what you're hoping to achieve, during this first session.
A physical assessment will then follow and include both active and passive movements, in order to ascertain the state of injury, restriction or muscular-skeletal disorder. Please wear loose-fitting clothes, shorts (and bra for women) and bring any relevant footwear (eg. running/tennis shoes).
Diagnosis & Treatment Plan
On the basis of both the consultation and physical assessment, I’ll then be able to provide a diagnosis and propose a treatment plan. This will include the treatment techniques to be used, why and to what effect. Objectives and outcomes for the treatment will be discussed and agreed by both of us prior to any treatment commencing.
Should treatment not be appropriate for any reason, I will explain this and refer you to the relevant practitioner.
I'll also advise on the likely number of treatment sessions needed, explain any rehabilitation exercises, as well as the process of injury recovery or performance optimisation. It's important that you understand all aspects of your treatment plan and are happy to proceed, so I welcome any questions during your treatment. I'm also available by phone or email should you have any further queries. Follow-up appointments, if necessary, can be arranged at the time of your initial treatment or at a time to suit you.
New Patient Consultation Form
If you are a new patient, there is a Consultation/GDPR form which should be completed and then brought with you to your appointment:
Due to Covid-19, I have instigated specific hygiene protocols which include two forms to complete and sign. Please ensure you have read my Physio Hygiene Protocol which explains procedure on arrival and confirms that you’re happy to have treatment under these measures. A signed copy of this, along with the Physio Consent and Screening form should be brought with you to your appointment or emailed back to me 24 hours in advance:
'All sessions take up to an hour and are payable at the time/day of treatment via cash, cheque, BACS bank transfer or via the website.
Acupuncture needles and Kinesio-tape will each incur an additional £3 charge which is payable at the time of treatment.
Please note that the full treatment fee will be charged if you fail to keep your appointment or cancel/amend with less than 24 working hours in advance (unless your treatment slot can be re-booked).
I do my best to re-book any late cancellation slots in order to avoid charging cancellation fees, but this is not always possible if less than 24 hours’ notice is given. Working hours’ notice is Monday-Friday 8am-6pm.
Appointments booked for a Monday, or a Tuesday following a bank holiday, should be cancelled or changed by midday on the Saturday prior.
